factors. It could be competition, monetary reward, encouragement,
status, job security, praise, fear or public recognition etc. There are
instances when a company has announced certain policy and as a result of
that, employees start feeling de-motivated. The survey should elicit
such fears and information.

motivation questionnaire. Factors like, patterns of motivation in the
company, employee empowerment, goals of the employees, opinion about the
company, involvement of employees in the growth and development of the
company and consistency of company’s internal image with its external
one etc. The findings of these entire questions in a well formulated
questionnaire can work as a wonderful tool in the hands of management to
identify the motivation level of the employees and help them in devising
subsequent steps that need to be taken.
